I would go see Gacek. Antivirals certainly can't hurt and they might end this before it gets bad. I also had hearing loss years before vertigo....

Benzos are known to cause hyperacusis. Not sure if you're taking any, but if you are, they might be the culprit.
I would personally stay on the highest dose until I got relief.
